Request for Job Attached Status (UIB-432)

After an employee has filed their initial unemployment benefits claim indicating "job-attached" status, employers will receive Form UIB-432 from the CO Dept. of Labor & Employment (CDLE) to confirm the status. You do not have to return Form UIB-432 if you intend to re-employ the individual within 16 weeks of the date of separation. However, if you do not intend to rehire the employee, then you should indicate as such and return to CDLE via mail.
