Employee Hiring Forms

Authorized Employer contacts without direct access, please submit new hire information using this secure online form; a printable version is available upon request. Please use our secure upload page to safely send employee information, such as new hire and direct deposit forms.  All submissions will be re-verified by your account manager to ensure they were approved by an authorized employer contact.
Hiring with HUB Employee HR Portal: If you are an admin user of the  HUB Employee Portal, you can send your newly hired employees their initial paperwork to complete using the HUB "new hire" feature. The HUB new hire feature will guide you and your employee through a secure and paperless process in order to set them up in our system for payroll processing. Please contact your account manager if the new hire feature is not enabled on your HUB account or if you are interested in learning about HUB timekeeping and HR solutions.

New Employees Should:

  1. Complete a W4 - Employee's Withholding Allowance Certificate.
  2. Complete Section 1 of the I-9 Employment Eligibility Verification and provide personal documentation to their employer or hiring manager 
  3. Present to their employer or hiring manager personal documents as explained on page 9 of the I-9 Form Instructions.
  4. Optionally, complete an Authorization for Direct Deposit of their paychecks.
  5. Be provided an ACA Health Coverage Notice unique to their employer's health insurance options.

Hiring Managers or Supervisors Should:

  1. Complete Section 2 of the I-9 Employment Eligibility Verification.
  2. Confirm the authenticity of the documents presented to them by the employee as explained on page 9 of the I-9 Form Instructions.
  3. Retain copies of the completed forms in your HR files; ASAP does not need all of them.
